We’re Here for You
Our patients and residents come to Providence Rest either through referral from a hospital, other health care facility or because they are looking for someplace trustworthy they can go to recover from an illness or injury. They also come to us when looking for a safe, secure and compassionate community to live their remaining days. While we make the admission process as easy as possible, certain paperwork is needed before you or your loved one is able to join us. If you or your loved one is coming to us from a hospital or other health care facility, they will provide us with the necessary paperwork. If you are taking this step on your own, most home care agencies can help you with the necessary forms.
Providence Rest is part of the Montefiore Health System SNF (skilled nursing facilities) Collaborative. A team of experts from Montefiore Medical Center carefully select nursing facilities for inclusion in this Collaborative, checking their safety records, reviewing how they care for patients with complex conditions and looking at information about their services, quality, and patient satisfaction.
